Notes
It is a Pumpkin that was carved out with a Kitchen Knife. To do this, use a Kitchen Knife, Whacking Driller of Fate, or Sneaky Stabber of Eliteness on it. After carving, you can use the Pumpkinhead on a Candlestick to transform it into a Jack-o'-lantern to provide a bit of light. 
There is a chance to obtain a Pumpkinhead when skinning the Slain Mutated Pumpkin within its portal.
