For non-boss creatures, this item is only dropped by players with at least one of the Goshnar's Taints active. For non-boss creatures and bosses with the exception of Goshnar's Megalomania, the drop chance is higher for players with more taints. The chance of dropping it from Megalomania increases every time you kill it and resets to the initial value whenever a Bag is dropped.
While you can not equip these bags, they are an exception in the Bosstiary's equipment loot bonus, and setting Soul War bosses in your boss slot will increase the chances of dropping it.
The bag quickly transforms into another item once the Reward Container it's in is opened. It can be transformed into any of the parts of the Soul Set:
